Description

The SNOMED code lists were created by compiling codes from the Quality and Outcomes Framework lists (e.g. QOF Dementia Cluster) and the NHS Digital Primary Care Domain Refsets. (e.g. for Parkinson's etc).

Methodology

Context

The codelist is for use in a neurodegenerative disease burden study (Bristol/ Edinburgh/ Cambridge) to categorise dementias by subtypes.

Inclusion/exclusion criteria

Codes indicative of dementia but without a specific subtype.

Borderline cases

NB. This codelist contains "in remission" codes, therefore, it is not suitable for identifying individuals with "active" dementia (e.g. those suitable for drug treatments). This decision was made because individuals coded as "in remission" should still be counted towards prevalence (as remission does not indicate resolution). Similarly, a decision was made to include "history of" codes as the codelist is primarily used for prevalence rather than incidence. Dementias are unlikely to resolve completely.

Codes Dementia due to disorder of central nervous system 724776007 (20 uses) & Dementia associated with another disease 191519005 (20k uses) are headings for dementia codes due to other specific disorders. However, by themselves, they do not indicate a specific pathology and would be unlikely to be coded if a specific defined aetiology was present. For this reason, they were included in the unspecified codelist. "Associated with another disease" may also be used in practice to reflect a co-morbidity rather than dementia cause.

Sensitivity vs specificity

Codelist is designed to be specific for dementia diagnosis (as opposed to "memory issues" or "cognitive impairment").
