Description

The SNOMED code lists were created by compiling codes from the Quality and Outcomes Framework lists (e.g. QOF Dementia Cluster) and the NHS Digital Primary Care Domain Refsets. (e.g. for Parkinson's etc).

Methodology

Context

The codelist is for use in a neurodegenerative disease burden study (Bristol/ Edinburgh/ Cambridge) to categorise dementias by subtypes.

Inclusion/exclusion criteria

Codes indicative of diagnosis of vascular dementia were selected as well as dementias due to neurovascular pathology. Shrip browser was used to search for additional codes. Comparison (diff) was made to the NHS ref set (DEMVASC_COD).

Borderline cases

NB. This codelist contains "in remission" codes, therefore, it is not suitable for identifying individuals with "active" disease (e.g. those suitable for drug treatments). This decision was made because individuals coded as "in remission" should still be counted towards prevalence (as remission does not indicate resolution; and resolution of dementia is unlikely).

Codes related to neurovascular disease resulting in dementia were included as by definition, these are specific subtypes of vascular dementia: Dementia due to cerebral amyloid angiopathy (1259488005) Dementia due to hypertensive encephalopathy (1259531005) HtrA serine peptidase 1-related autosomal dominant cerebral small vessel disease (1186724002)

The exception to this were inflammatory disorders of cerebral vasculitis, which differ from the "typical" atherosclerotic or ischaemic vascular dementias. Dementia due to cerebral vasculitis (1259485008) Dementia due to polyarteritis nodosa (1259663006) Dementia due to systemic lupus erythematosus (1259517005) These were classified under dementias "other specified".

Sensitivity vs specificity

Codes specific for vascular dementia and dementia caused by neurovascular disease were selected.
